For parents of child-clients:
If instructed, please, print and complete the following packets. The information from these packets will help paint a clearer picture of your child's background and current issues.
The School Problems Checklist is to be completed by one or more of your child's teachers. The teacher may either give the completed checklist back to you or fax it directly to me at the number listed on the form. This does not have to be completed prior to our first appointment, but not too long afterwards.
ADULTS
For Adults coming in to be tested for ADD or ADHD, please print the following two packets. Complete the Adult ADHD Interview and bring it with you to our first appointment. The Adult ADHD Collateral Information packet is to be given to a parent, prior caregiver, or other person, who was an adult and knew you very well when you were a child. The Collateral Information packet does not need to be completed by our first appointment.
